Key terms used in these tools
Care and support needs are needs related to physical or mental impairment or illness that affect daily living, independence, safety or wellbeing. Cumulative harm is the combined impact of repeated or connected concerns over time. Shared formulation is an evidence-based explanation of what is happening, why, what may happen next and what remains uncertain. Impact is the difference an action makes—not simply confirmation that the task was completed.
